FUNDING: Initiative to Reduce Avoidable Hospitalizations - April 3 Webinar

$128 million CMS initiative to reduce hospitalizations among nursing home residents.

4/3/12 Webinar to discuss the initiative and provide an overview of the new Initiative and describe the application process for prospective participants. (call-instructions in CMS announcement pasted below).  They will also archive the slides and presentations at

 

4/30/12 Notice of intent to apply due.

 

 

On page 9 of the proposal it mentions "Coordinate and improve management and monitoring of prescription drugs to reduce risk of polypharmacy and adverse drug events for residents, including inappropriate prescribing of psychotropic drugs" and "Enhanced behavioral health assessments, treatment, and management."  Eligible organizations can include physician practices, care management organizations, and other entities. Both for-profit and not-for-profit organizations are eligible to apply. It requires applicants to "Coordinate closely with State Medicaid and State survey and certification agencies and State public health and health reform efforts, including other CMS demonstrations and waivers…

 

I can't tell exactly who would be likely to get such a grant, but thought I would pass along.

Initiative to Reduce Avoidable Hospitalizations Among Nursing Facility Residents Webinar

 

On Tuesday April 3rd, the Center for Medicare & Medicaid Services' Medicare-Medicaid Coordination Office and Innovation Center invite you to participate in a webinar to discuss the Initiative to Reduce Avoidable Hospitalizations Among Nursing Facility Residents. The webinar will provide an overview of the new Initiative and describe the application process for prospective participants. 

Through the Initiative, CMS will support organizations that will partner with nursing facilities and States to provide coordinated, person-centered care for long-stay nursing facility residents who are enrolled in the Medicare and Medicaid programs. Eligible organizations can include physician practices, care management organizations, and other entities. Both for-profit and not-for-profit organizations are eligible to apply. More information will be provided during the webinar.

On Tuesday, April 3, 2012, staff from MMCO and the Innovation Center will be hosting a webinar to provide an overview of the Initiative and application requirements.
